Under the EU General Data Protection Regulation (GDPR) and the Data Protection Act 2018, individuals have the right to request access to, or a copy of, personal data that an organisation holds about them. This includes all computerised data, health records, staff records, volunteer records and donor records.

The Hanham Health Subject Access Request Policy details our responsibilities to comply with this legislation and outlines our processes for doing so. A copy of this policy is available upon request.

Patients requesting access to their medical records are asked to complete the Subject Access Form. Requests will be processed and completed within 1 calendar month, unless deemed to be complex; in this situation, the patient will be notified that the request will take longer to process.

Patients will not be charged for access to their medical records, unless the request is deemed to be excessive or repetitive; in this situation, we will do all we can to provide the necessary information without charge, but may apply a small admin charge in exceptional circumstances.

Subject Access Request Forms can be submitted via the following methods:

- Hand directly to the receptionist at the surgery

- Post to: Almondsbury Surgery, Sundays Hill, Almondsbury, Bristol, BS32 4DS
